Most people who grind their teeth at night have no idea they do it. The signs show up elsewhere: a sore jaw in the morning, headaches around the temples, teeth that ache to cold, and front teeth that are flattening at the edges.

Grinding is common and largely harmless in the short term. Over years it is one of the most destructive things that happens to teeth, and it happens quietly. Here is what to look for.

The Signs You Can Notice Yourself

Waking With a Sore Jaw or a Headache

The muscles that close your jaw are among the strongest in the body, and clenching all night exhausts them. A dull ache at the angle of the jaw or a headache in the temples that is worst on waking and eases through the morning is the classic pattern.

Teeth That Are Sensitive in the Morning

Pressure and grinding wear through enamel and expose dentin. If cold bothers you most first thing and settles later, grinding is a strong suspect. This is a different pattern from ordinary cold sensitivity, which does not follow the clock.

Flat, Even Edges on Your Front Teeth

Healthy front teeth have a slightly scalloped edge. If yours look level, as if filed straight across, that is wear. Compare them to a photo from ten years ago if you have one, the change is usually obvious.

Small Chips and Hairline Cracks

Enamel that has thinned chips at the edges. Fine vertical lines in the front teeth catch the light. Neither hurts, which is why they go unnoticed.

A Ridge Along the Inside of Your Cheek

Run your tongue along the inside of your cheek at the level where your teeth meet. A raised white line there is from cheek tissue being pressed against teeth all night. Scalloped edges on the sides of the tongue mean the same thing.

Grinding is loud. If a partner mentions it, believe them.

Why It Happens

Stress and anxiety are the most common triggers, and grinding often tracks with stressful periods rather than running constantly. Caffeine late in the day, alcohol in the evening and nicotine all increase it.

The one worth taking seriously is sleep apnea. When the airway narrows during sleep, the jaw muscles activate as part of the effort to reopen it, and grinding is often the visible side of that. If grinding comes with snoring, morning headaches and daytime tiredness, look at the warning signs of sleep apnea before assuming it is stress.

Some medications, particularly certain antidepressants, list it as a side effect. A misaligned bite plays a smaller role than people assume.

What It Costs If You Leave It

Enamel does not regenerate. Once a tooth is flattened, that height is gone, and the shortening changes how your face sits over decades. Cracked teeth need crowns. Fillings and crowns wear through faster than they should. Gums recede at the points taking the most force, exposing roots. Jaw joint problems develop in some people, with clicking, limited opening and pain.

The treatment at the early stage is a night guard. At the late stage it is rebuilding multiple teeth, and the difference in cost between those two is not small.

What Helps

A custom night guard is the main answer. It does not stop the grinding, it gives the force somewhere harmless to go, and a piece of acrylic wearing down instead of your enamel is a good trade. Custom guards are thinner and more comfortable than the boil and bite kind, and they do not shift your bite.

Beyond that: cut caffeine after mid afternoon, cut alcohol close to bedtime, and treat the sleep apnea if that is what is driving it. Jaw stretches and warmth on the muscles help the soreness without touching the cause.

What a Night Guard Is Actually Like

A custom guard is made from an impression or a scan of your teeth, so it clips into place rather than sitting loosely. Most people describe the first three or four nights as odd and then stop noticing it.

It does not stop the grinding. It gives the force something disposable to land on. A guard wearing thin over two or three years is a guard doing its job, and replacing it is far cheaper than replacing enamel.

Boil and bite guards from a pharmacy are thicker, wear through in months, and if they fit badly they can move teeth slightly over time. They are a reasonable stopgap while you get a proper one made, not a long term answer.

What Grinding Costs Over Ten Years

Enamel wears at a rate you never notice month to month. Over a decade of untreated grinding it is common to see front teeth shortened by a millimetre or more, chips along the edges, cracked back teeth needing crowns, and gum recession at the pressure points. Rebuilding worn teeth means crowns or veneers across several teeth at once, which is the expensive end of dentistry. A guard is a fraction of that.
